Select this option to consult with a licensed medical provider about a general health concern, laboratory request, or prescription renewal request. Any diagnosis, lab order, prescription, or treatment recommendation is determined solely by the provider and is not guaranteed.

Consultation With a Standalone Treatment Request — $75
Select this option when requesting a provider evaluation for a specific treatment that is not included with another coordinated care program. For example, select this option when requesting Metformin, Trimix Rescue etc. separately rather than as part of a weight-management or sexual wellness program. Treatment approval is based solely on the licensed provider’s independent medical determination.
