General Care Consultation Categories & Medication Reference
Our General Care Consultation provides a convenient way to connect remotely with a licensed medical provider for eligible general health concerns, laboratory requests, prescription renewals, and other treatment requests. Through our care coordination services, we help make accessing appropriate care simple and convenient.
General Care Consultations are intended for treatment requests not already offered through a Balanced Life Care Curated Care Plan. They may not be used to request treatments available through our weight loss, testosterone, men’s sexual wellness, hormone replacement therapy (HRT), or other Curated Care Plans, including treatments such as phentermine and Trimix. If the treatment you are requesting is offered through a Curated Care Plan, please select the applicable treatment request instead.
Prescriptions, laboratory orders, and treatment approvals are not guaranteed. All medical decisions are made independently by the licensed medical provider based on their professional medical judgment and your individual medical circumstances.

They do not constitute a prescribing protocol or guarantee that a medication will be prescribed. Diagnosis, medication selection, dosing, testing, contraindication review, and follow-up are determined independently by the licensed medical provider.

General Care Consultation — $120
Select this option to consult with a licensed medical provider about a general health concern, laboratory request, or prescription renewal request. Any diagnosis, lab order, prescription, or treatment recommendation is determined solely by the provider and is not guaranteed.
Lab Order Request $120
Select this option to consult with a licensed medical provider about a laboratory testing request. The $120 fee covers the provider consultation only and does not include laboratory testing fees. The provider will determine which laboratory tests, if any, are medically appropriate.
Balanced Life Care coordinates laboratory testing through Quest Diagnostics. Once an order is issued, you may choose one of the following payment options:
Using Insurance: Balanced Life Care does not accept or bill insurance. If you choose to use insurance for your laboratory testing, you are responsible for confirming coverage and Quest Diagnostics’ participation with your insurance plan. You will present your insurance information directly to Quest Diagnostics when completing your lab work. Any charges not covered by your insurance are your responsibility.
Self-Pay Through Balanced Life Care: If you choose not to use insurance, Balanced Life Care can coordinate your laboratory testing on a self-pay basis. The cost will be based on the specific tests ordered by the provider and will be disclosed before payment. Balanced Life Care will collect the applicable laboratory payment and coordinate the order with Quest Diagnostics through our care coordination services.
Laboratory orders are not guaranteed and are issued solely at the licensed medical provider’s independent medical discretion.
Medication and laboratory costs are separate from the General Care Consultation fee.
Laboratory costs vary depending on the specific tests ordered by the medical provider. Once the recommended testing is determined, the applicable laboratory costs can be provided. Patients may contact their insurance carrier to determine whether the ordered testing is covered under their plan or may elect to have Balanced Life Care coordinate self-pay laboratory pricing and provide the cost before testing is completed. Balanced Life Care does not accept or bill insurance.
If medication is prescribed, the prescription may be sent to the patient’s pharmacy of choice. When available, patients may instead elect to have Balanced Life Care coordinate pharmacy services. When Balanced Life Care coordinates medication fulfillment, the prescription will be fulfilled through an appropriately licensed pharmacy serving the patient’s state, and the medication will be shipped directly to the patient’s home or another eligible physical address designated by the patient. Medication pricing will be communicated separately from the consultation fee.
Depending on the condition or treatment requested, the provider may require laboratory testing, current vital signs, prior medical records, or referral to another healthcare provider or an in-person healthcare facility before treatment can be provided.

Can I get a prescription through a consultation?
A prescription is not guaranteed. A licensed healthcare professional will review your symptoms, medical history, current medications, allergies, and other relevant information to determine whether prescription treatment is appropriate.
Do I need a consultation before requesting medication?
For medications that require clinical evaluation, a healthcare professional must determine whether the medication is appropriate for you. The evaluation may include questions about your health history, current medications, allergies, symptoms, and other relevant factors.
Will I need laboratory testing?
Some conditions or medications may require laboratory testing before treatment or during follow-up. Your healthcare professional will determine whether testing is appropriate based on your individual circumstances.
Are prescriptions, laboratory orders, or treatments guaranteed after a consultation?
No. A consultation does not guarantee a prescription, laboratory order, diagnosis, or treatment. These decisions are made solely by the licensed medical provider based on their clinical assessment and the information provided during the consultation.
What happens after my consultation?
After your consultation, the licensed medical provider will determine the appropriate next steps based on your individual assessment. This may include a prescription, laboratory order, treatment recommendation, or additional guidance when medically appropriate.
Can I use insurance for my laboratory testing?
Balanced Life Care does not accept or bill insurance. If you choose to use insurance for your laboratory testing, you are responsible for confirming your coverage and whether Quest Diagnostics participates with your insurance plan.
Can I request STI testing or discuss sexual health concerns?
Yes. Sexual health and STI concerns can be discussed with a licensed medical provider. Depending on your individual concern and clinical assessment, testing may include chlamydia and gonorrhea, HIV, trichomoniasis, or other site-specific testing when appropriate.
Does the $120 General Care Consultation for a Lab Order Request include laboratory testing?
No. The $120 fee covers the General Care Consultation and provider review for your laboratory request. Laboratory testing fees are separate and vary based on the specific tests ordered by the provider.
Once your laboratory order is determined, you may contact your insurance carrier to verify coverage or elect to have Balanced Life Care coordinate self-pay laboratory pricing and provide you with the cost. Balanced Life Care does not accept or bill insurance.
