Connect with a licensed medical provider from the convenience of home to discuss a general health concern, laboratory request, or prescription renewal request. Following an independent medical evaluation, the provider will determine whether any testing, prescription, or treatment is medically appropriate. Prescriptions, laboratory orders, and treatment approvals are not guaranteed.

General Care Consultation — $120
General Care Consultations cannot be used to request treatments offered through a Balanced Life Care Curated Care Plan, including weight loss, testosterone, sexual wellness, or hormone replacement therapy (HRT) treatments. If your requested treatment is offered through a Curated Care Plan, you must select the applicable treatment request instead.
Select this option to consult with a licensed medical provider about a general health concern, laboratory request, or prescription renewal request. Any diagnosis, lab order, prescription, or treatment recommendation is determined solely by the provider and is not guaranteed.
Consultation With a Standalone Treatment Request — $75
Select this option when requesting a provider evaluation for a specific treatment that is not included with another coordinated care program. For example, select this option when requesting Metformin, Trimix Rescue etc. separately rather than as part of a weight-management or sexual wellness program. Treatment approval is based solely on the licensed provider’s independent medical determination.
Description
The General Care Consultation is intended for eligible general health concerns, laboratory requests, prescription renewal requests, and other treatment requests that are not already offered through a Balanced Life Care Curated Care Plan.
A General Care Consultation may not be used to request a treatment or medication that Balanced Life Care offers through a designated Curated Care Plan. This includes, but is not limited to, weight-loss treatments such as phentermine, testosterone replacement therapy (TRT), TriMix and other sexual wellness treatments, hormone replacement therapy (HRT), and other treatments offered through our Curated Care Plans.
If the treatment you are seeking is available through a Curated Care Plan, you must purchase the applicable treatment request rather than a General Care Consultation.
Purchasing a consultation or treatment request does not guarantee a prescription or treatment approval. All medical decisions, including diagnosis, treatment selection, prescribing, dosage, and eligibility for treatment, are made independently by the licensed medical provider based on their professional medical judgment.
